when we need social care support it can feel isolating and confusing. trying to identify what care you need and how to engage services can be a frustrating and time consuming exercise. ​ usually we are at breaking point when we ask for help, and things can quickly and easily spiral out of control with older adults being placed in residential or nursing homes as families are unsure where else to turn. we aim to support older adults and their families through a free initial consultation to identify whether our service is the most suited and if not signpost to alternate services. we are also able to complete a comprehensive therapeutic assessment, to identify an individual’s needs and strengths with the aim of crafting a care plan as individual as you are. ​ we support individuals and their families in identifying and engaging services, such as care at home as well as technologies such as “lifeline” alarm systems to support and meet the identified needs whilst also reducing risk. if in th
