social prescribing is a way of linking people who commonly access primary care settings with non-medical sources of support within their community. it provides a more holistic response to people’s needs, to improve people’s wellbeing and health, and impacts the wider determinants of health such as debt and housing. social prescribing can reduce time spent with clinical providers, building resilience and making a positive difference to people in the local community. we understand that life has many challenges and we strive to help others implement positive changes that make a real difference to everyday wellbeing and mental health; we walk the journey alongside people to connect to community sources of support. the social prescribing portsmouth service is delivered by the you trust and has a number of stakeholder partners. it is a 4 year funded programme supported by the department of health and social care, portsmouth ccg and portsmouth city council. our vision is for everyone who is
