socially determined is a healthcare analytics company based in washington dc. we are developing the science of the social determinants of health and partnering with our clients to implement community-based intervention programs. we believe that social determinants drive over 50% of our health outcomes and that we can understand those determinants using advanced analytics and data science. highlighting and quantifying these factors will allow us to design targeted interventions programs that improve healthcare in our communities. we are a team of physicians and engineers - all experienced entrepreneurs. we are actively building our data science team and consulting force and are looking to build a multi-disciplinary team that is passionate about healthcare and making a difference in our communities. we are socially determined.
