Southern Prairie Community Care

southern prairie community care (spcc) is collaboration between 12 counties located in southwestern minnesota. the participating counties of spcc are: chippewa, cottonwood, jackson, kandiyohi, lincoln, lyon, nobles, murray, redwood, rock, swift and yellow medicine. spcc was formed in july 2012 as a joint powers entity. our counties share similar beliefs and values and a vision to improve the quality of life for their residents. we also share higher than state average rates of poverty, obesity, physical inactivity, diabetes and other health indicators. because of this, we have come together to improve the health and wellness of our residents. our mission is to enhance the quality of life for our citizens through facilitating the integration of services and supports provided throughout our community. we believe that by better aligning the services and supports that our residents receive from providers, county services (broadly defined to include public health, human services, social serv
