southland geriatric and palliative care (sgpc) is a leading network of providers including physicians, nurse practitioners, physician assistants and specialists providing home based primary care to seniors throughout the community. we visit patients in assisted/independent living communities, memory care, and private homes. we collaborate with leading mobile services including radiology, laboratories, pharmacies and dme to ensure patients get the most comprehensive and coordinated care in the comfort of their homes. no more arranging transportation or long waits at the doctor’s office because, “we take the wait out of the waiting room.” patients spend more time with their loved ones and less time coordinating primary care needs.
