Star Family Health Team

family health teams are a key piece in building a health care system that promotes good health, reduces wait times and improves access to care. a family health team is an approach in primary health care that brings together different health care providers to co-ordinate the highest possible quality of care for the patient. the star family health team was established in fall 2006 providing service to stratford, tavistock and region (star), having two clinics in stratford and one clinic in tavistock. mission: to assist health service providers to function within a collaborative interdisciplinary setting in order to enhance their ability to deliver effective, appropriate, and quality health care services for patients. www.starfht.ca
