de stichting zorg in houten is verantwoordelijk voor het beleid en de ontwikkeling van de ketenzorg en geïntegreerde eerstelijnszorg in de 5 eerstelijns medische centra (emc). in de 5 emc’s komen verschillende zorgdisciplines samen en vormen een multidisciplinair zorgcentrum. ons beleid is erop gericht de samenwerking tussen de zorgdisciplines te optimaliseren en zodoende de beste geïntegreerde zorg aan te bieden, dichtbij huis. steeds meer mensen hebben last van een chronische aandoening en hebben daarom geïntegreerde zorg nodig. dit is zorg waarbij meerdere zorgverleners zijn betrokken om de aandoening te behandelen en de ziektelast te verlichten. denk daarbij aan samenwerking tussen huisarts, fysiotherapeut, apotheek, psycholoog en wijkverpleging. door samenwerking tussen de zorgverleners te stimuleren streven we naar zorg op maat, afgestemd op de behoefte van de patiënt. een goed voorbeeld hiervan zijn de zorgprogramma’s en leefstijlmodules die we hebben ontwikkeld.
