the eastern health alliance is the regional health system for the people of eastern singapore. officially launched on nov 18, 2011, it is in line with the national direction to make healthcare ongoing rather than episodic especially for people with long-term chronic conditions like diabetes, stroke, cancer, lung and heart disease. singapore's population is ageing and the prevalence of such conditions is growing, especially among the elderly, making patient care more complex. the eastern health alliance seeks to gradually reshape current models of healthcare to meet these challenges. guided by its vision "towards seamless quality care – together", eastern health alliance members are focused on progressively delivering - through their combined expertise - an integrated range of healthcare services, from disease prevention, early detection and treatment, through to ongoing care, providing the right quality care at the right time for each patient. the overall objective? to make each pa
