eastern sydney medicare local (esml) is one of 61 primary health care organisations introduced under the australia wide national health reform. we're funded by the department of health and ageing and exist to improve the health of individuals within the community by making local health care easier to access, identifying where local communities are missing out on the services they need and coordinating services to address these gaps. the esml region has a population of approximately 400,000 residents, covering 5 local government areas, including; parts of sydney cbd, randwick, botany bay, waverley and woollahra, as well as lord howe island. our core objective is to improve the primary health care journey for patients and health professionals throughout the region by; - assisting with the development of integrated and coordinated services from the general practice setting through to allied health services and hospital services, - providing support to clinicians and service providers, - i
