Kansas Patient Centered Medical Home

the patient centered medical home (pcmh) is a health care delivery model focusing on the relationship between a patient and their personal physician. the model is designed for patients to receive the right care at the right time, lowering their health care costs and improving their overall health across the health care system. the kansas chapter of the american college of physicians, kansas chapter of the american academy of pediatrics, the kansas association of osteopathic medicine, and the kansas medical society have worked in coalition with kansas academy of family physicans to develop the kansas pcmh initiative. the kansas initiative will span three years concluding in 2013. funding for this project was provided in part by the united methodist health ministry fund, a philanthropy based in hutchinson, kansas; sunflower foundation: health care for kansans, a topeka-based philanthropic organization with the mission to serve as a catalyst for improving the health of kansans; and the ka
